Information for GPs
Admitting a patient
Phone our Hospital Co-ordinator on 03 9468 0804 and provide the following details:
- Patient's name
- Address
- Contact numbers
- Health insurance details
- Date of birth
- Outline of current issues
- Current regime
- Previous treatments
- Any risk history or potential risk factors
To ensure the patient has adequate cover, the co-ordinator will take the patients details, ensure a health fund check is done and will contact you to discuss the admission.
Step 1
Business Hours
Contact our Intake Service on 03 9468 0850 and provide the Intake Clinician with the following information:
- Patient's name
- Address
- Contact numbers
- Health insurance details
- Date of birth
- Outline of current issues
- Current regime
- Previous treatments
- Any risk history or potential risk factors
Step 2
Fax your referral letter to 03 9468 0358.
Step 3
The Intake clinician / Coordinator will contact duty psychiatrist, registrar or manager of specialist program to discuss the referral.
Step 4
Intake clinician will contact patient and any additional information will be obtained (including current risk assessment). The patient will also be informed of the outcome of their health fund check.
Step 5
Intake Clinician will discuss the following details with the referrer:
- Suitability of referral according to entry criteria of program
- Patient's health insurance status
If admission criteria is met and the psychiatrist is willing to admit, the patient will be contacted to arrange admission time.
Step 6
The referring doctor will be notified of patient's admission date and their treating psychiatrist.
Step 7
Following a patient discharge, a medical discharge summary will be sent to the referrer.

Fax your referral letter to 03 9468 0120 addressed to “Dear Psychiatrist” at Northpark Private Hospital Consulting Suites.
Referrals should include:
- Patient's name

- Date of birth
- Outline current issues
- Current regime
- Previous treatments
- Any risk history or potential risk factors

Step 2
The allocated consultant psychiatrist’s secretary will contact the patient and referring doctor with the appointment details. The patient will be seen at:

Step 1
Fax your referral to (03) 9468 0358 marked to the attention of the Day Program Manager and address the referral to 'Dear Doctor' at Northpark Private Hospital. Referrals should include:
- Patient's name
- Address
- Contact numbers
- Health insurance details
- Date of birth
- Outline current issues
- Current regime
- Previous treatments
- Any risk history or potential risk factors
Step 2
The Day Programs Manager will contact you to discuss the following:
- Suitability of referral according to entry criteria of program
- Patient's health insurance status
Step 3
Following patient's discharge from the program, a discharge summary will be sent to the referrer.
